The purpose of Flight Safety reporting is to alert all concerned of circumstances that have resulted in or have the potential to cause damage to aircraft, air weapons or injuries to personnel. Flight Safety occurrence reporting and investigation allow appropriate Preventive Measures to be identified and implemented by command authorities.
Flight Safety reports are only used for minimizing the risk inherent to flight operations and help in the prevention of future accidents. Information provided and contact details will always be treated in the strictest confidence. The voluntary inclusion of contact details will help the Flight Safety staff to follow up the investigation and provide feedback.
A CF 215 Flight Safety Occurrence Initial Report shall be submitted to a Flight Safety Officer within 12 hours of an occurrence involving the security of a Canadian Forces aircraft. The CF 215 format is detailed at Annex E to Chapter 9 of A-GA-135-001/AA-001. The following guidelines should be applied to ascertain if an event should be reported as an FS occurrence:
Was there an injury or illness to personnel engaged in or supporting air operations?
Was there damage to a CF-owned aircraft or aircraft operated by or on behalf of CF / DND?
Was there damage to CF equipment used to support air operations?
Was there injury or potential for injury to civilian personnel?
Was there damage or potential for damage to civilian property or to the environment ?
Could reporting the FS event generate a preventive measure that may prevent a similar occurrence?
A DND 2484 Flight Safety Hazard Report should be filed when personnel identifies any condition t hat has the potential to cause injury or damage during Canadian Forces air activities or support to the latter. The DND 2484 format is detailed at Annex A to Chapter 7 of A-GA-135-001/AA-001.
